Providing Telephone Numbers and Duty to Notify
By providing your mobile number to Legacy Health & Wellness, you confirm that the number is accurate and that you are the current subscriber or owner of the provided telephone number. If your contact information changes, including ownership of your mobile number, you agree to notify us immediately before the change takes effect by emailing us at [email protected].
Your Consent to Receive Automated Calls/Texts
By voluntarily providing your telephone number(s), you expressly consent to receive recurring automated text messages (SMS, MMS, or similar technologies) from Legacy Health & Wellness regarding our products, services, promotions, appointment reminders, billing, and other informational updates. You understand that consent is not required to make a purchase. Message frequency may vary, and message and data rates may apply based on your mobile carrier.
Opt-Out Instructions
Your consent to receive automated texts is voluntary, and you may opt-out at any time.
To stop receiving text messages, reply with STOP, END, CANCEL, UNSUBSCRIBE, or QUIT to any message from us.
You may also request to opt out by emailing [email protected] with “Opt-Out Request” in the subject line.

You acknowledge and agree to receive a final text message confirming your opt-out request.
Indemnification to Us
You agree to indemnify and hold Legacy Health & Wellness harmless from any privacy, tort, or other claims, including but not limited to claims under the Telephone Consumer Protection Act (TCPA) or any equivalent state laws. This includes any claims arising from your voluntary provision of a telephone number that is not owned by you and/or your failure to notify us of a number change. You agree to indemnify, defend, and hold us harmless from and against any claims, losses, liabilities, costs, and expenses (including attorneys’ fees) arising from such issues.
Participation Requirements
By participating in our SMS service, you confirm that you are at least 18 years old and that you:
- Own a mobile device capable of receiving text messages.
- Use a participating wireless carrier (not all providers support this service).
- Are a wireless service subscriber with an active text messaging plan.
Please check with your mobile carrier to determine if your phone supports text messaging services.
Miscellaneous
You represent and warrant that you have all necessary rights, power, and authority to agree to these Terms & Conditions and to fulfill your obligations hereunder. If any provision of this agreement is found to be unenforceable or invalid, that provision will be limited or removed, and the remaining terms will continue in full force and effect.
